Before coming to your appointment print and fill out the Medical History Form and bring it with you to your appointment.
Please assist us by ensuring that the following information is available at the time of your consultation.
- Your referral slip and any X-rays, if applicable • A list of medications you are presently taking
- If you have dental insurance, bring the applicable card. This will save time and allow us to help you process any claims.
The fee for your endodontic treatment will be based on the extent of treatment. When you call to make an appointment we will discuss the fees involved. However, this is only an estimate and you will be charged accordingly when treatment is complete.
We accept an assignment of benefits from your insurance company with the following provisions. It is important to understand that the contract regarding your dental benefits is between you, your employer, and your insurance company. The obligation you have with our practice is to be responsible for the cost of your dental treatment, regardless of the amount that may or may not be reimbursed by your insurance company. Your dental insurance plan may or may not cover the full cost of your dental treatment.
For your convenience, we accept Visa, MasterCard and Debit. Personal cheque and cash will not be accepted. Please call our office for more details.
Cancellation and Missed Appointments:
When you book an appointment with us, we reserve that time specifically for you. As such, we require A 2 business days in the event an appointment must be cancelled. This allows other patients awaiting treatment to be rescheduled into the time slot initially reserved for you. Short notice cancellations (i.e. less than 2 business days or cancelling appointment on the answering machine) and missed appointments will be subject to fees payable.
